Allergist and immunologist Dr. Steve Dorman joins Dr. Michael Koren to discuss the causes, complications, and carousel of chronic cough. Dr. Dorman reviews what chronic cough is and how it may be due to anatomical changes, acid reflux, allergies, and so on. Because of this, the doctor explains, sufferers of cough may end up going around the "carousel" of chronic cough, where they end up seeing their primary care specialist, a GI doctor, an Ear, Nose & Throat (ENT) specialist, pulmonary physicians, and an allergist and immunologist like himself. The duo then tackle what can be done, from medications, new research, and non-medicinal treatments.

OBGYN Dr. Richard Myers joins Dr. Erich Schramm to discuss the name change from Polycystic Ovarian Syndrome (PCOS) to Polyendocrine Metabolic Ovarian Syndrome (PMOS). This syndrome is the combination of three parts: increased polyendocrine hormones (like insulin and androgens), changing metabolism, and differences in ovarian function Dr. Myers explains that the name change to PMOS reflects the changing understanding and focus of the syndrome from a purely ovarian-centered approach to a more holistic, full-body treatment regimen. The doctors also discuss how different patient desires drive different treatment options and the need for individualized, patient-centered care. They finish by looking at new treatments, including GLP-1 and GIP medications that can help address the underlying metabolic components of the PMOS.

Dermatologist Dr. Michael Bernhardt joins Dr. Mitchell Rothstein to discuss the rapidly changing and improving world of chronic skin condition. The two doctors discuss the correlation between chronic skin conditions and allergies, and the role that inflammation play in both. The docs then dive deep into our inflammation system.
This informs the next section of the podcast, where the Drs. Bernhardt and Rothstein talk about the landscape of chronic skin condition medications. They explain that these conditions had poor outcomes and ineffective medicines for decades, and that this picture has only changed in the last 4-5 years with new medications that work from the inside out instead of the outside in. They talk about new generations of medication that target the inflammatory signals themselves inside the body, showing rapid relief to otherwise untreatable conditions. They finish with a brief look at what is coming down the pipeline of clinical trials.

Nearly a decade after the last major cholesterol guidelines were released, the new 2026 Multi-Specialty Lipid Guidelines introduce significant changes in how cardiovascular risk is assessed and managed. In this episode, Albert Lopez, DO, FACP, FASPC, explains why LDL cholesterol treatment goals have returned, how newer markers such as ApoB and Lipoprotein(a) are reshaping risk assessment, and why earlier, more aggressive treatment may help reduce the world's leading cause of death, cardiovascular disease.
Dr. Al Lopez, DO, discusses the role of statins, non-HDL cholesterol, triglycerides, genetic cholesterol disorders, screening recommendations for children and older adults, and the importance of a team-based approach to prevention. Whether you're a healthcare professional or someone interested in protecting your heart health, this episode provides a practical overview of the latest evidence-based recommendations for preventing heart disease.

In this episode of MedEvidence, host Dr. Michael Koren speaks with epidemiologist Dr. Hannah Gardener about how everyday exposures may influence long-term health outcomes.
Dr. Gardener shares her journey from neuroscience and epidemiology training at Harvard to her current work studying stroke, dementia, and environmental risk factors. She explains how personal life experiences sparked her interest in environmental health and led her to examine how early-life and daily exposures can shape disease risk across the lifespan.
The conversation explores her research on diet soda consumption and its surprising associations with vascular outcomes, including stroke, heart disease, and cognitive decline. She breaks down what “prospective cohort studies” really mean and how to interpret findings without overgeneralizing results.
The discussion then expands to PFAS, often called “forever chemicals”, which are widely used in consumer products and persist in the environment and human body. Dr. Gardener explains where these chemicals are found, what health effects have been observed in the research, and practical, evidence-based strategies individuals can use to reduce exposure.
This episode offers a thoughtful, research-driven look at how environmental factors, diet choices, and chemical exposures intersect with long-term brain and cardiovascular health—and what can realistically be done today to reduce risk.
